Layout and Print Questionnaires and Reply Envelopes

You or the Survey Consultant would layout the questionnaire and print the survey booklets. We recommend a 20 percent overage. This overage may seem excessive at first, but shipments get lost or damaged and you need to respond with additional surveys on short notice. Depending on your method of administration, you may need outer mailing envelopes, as well as pre-addressed, postage-paid return mailers. Materials will be shipped as specified by the Steering Committee.

If you use a Diskette-By-Mail (DBM) survey, you or the Survey Consultant would contract with a service bureau for diskette duplication, labeling, and packaging.
