ERRATA, ADDENDA and EXOTICA

ERRATA:

1.  Question C.03.e.08.  The phrase "regular full time" should be "regular part
time".  Many of you have already picked this up and interpreted it correctly
based on the flow of the questions.

2.  Question F.02.a.02. and elsewhere.  The 3rd choice, 'Some locations but not
company headquarters," is duplicated at number 4.  Number 4 should be answered
as if it reads "Company headquarters and some locations" as it does in the
hardcopy.  Many of you have already picked this up and interpreted it correctly
based on the version in your hard copy.

3.  A VERY IMPORTANT QUESTION IS MISSING.  Question F.08.c.  What was the average
number of weeks of Job-Guaranteed time off for childbirth taken by Mothers in the
past year?   This question is missing from the Internet version, but is on your
hardcopy .PDF version.  Please answer this question in the comments for section
'F'.  If we find that the qnswer to this question is missing from your
application, we will call you to advise yuo.  Is it acceptable for you to submit
incidental corrections to us by email even after you submit your application.
